We are actively searching for skilled candidates to join our team as a Licensed Practical Nurse (LPN). In this pivotal role, you will coordinate healthcare needs for individuals facing behavioral health challenges. Your responsibilities will include delivering care through the nursing process, which encompasses planning, implementation, and education for residents and their families.

As an LPN, you will:

Instruct, counsel, and support designated participants, prioritizing their independence and unique needs.

Create and maintain an environment that fosters proactive decision-making and personal growth for each participant.

Assist psychiatrists and team members in addressing both behavioral and physical health requirements of program participants.

Support the Clozaril Clinic operations to streamline information exchange between psychiatrists and staff.

Collaborate with fellow nurses to execute nursing duties and maintain comprehensive documentation within the clinic.

Develop and oversee systems to ensure participants receive all necessary laboratory tests and that results are analyzed by the prescribing psychiatrist.

Qualifications:

Successful completion of a certified nursing program, demonstrated by an active LPN license.

Experience in community nursing, especially in mental health and co-occurring disorders.

Previous experience addressing psychiatric needs in a mental health setting.

Comprehensive understanding of psychiatric clinical practices and psychosocial rehabilitation.

Strong interpersonal, organizational, and analytical abilities.

Availability for emergency on-call duty.

Excellent written and verbal communication skills.

Valid driver's license is required.

Ability to work independently as well as part of a dynamic team.

Scope of the Role:

Ability to lift participants of varying weights while ensuring their comfort and safety.

Frequent communication through talking and listening.

Approximately 75% of the role will require community-based work activities.

Capability to perform crisis intervention during emergencies.

Willingness to report for duty during emergencies, including severe weather conditions.
