Department Description:
The Advanced Care at Home (ACAH) service line provides care for our patients in their home, in lieu of hospitalization. Patients receiving ACAH care are provided with a technology setup (installed by the team) that provides them with direct access to 24/7 virtual care from our central hub of nurses and physicians, and care services are delivered directly to the patient in their home, based on each patient’s needs.
The Design & Delivery Consultant IV position will work with the Roseville, Sacamento and South Sacramento Medical Centers, with potential expansion to additional medical centers in the South Bay over time. Key responsibilities of the role include:
Educate and support Medical Centers teams in the identification, screening, and enrollment of patients into ACAH
Responsible for the development, refinement, and delivery of educational materials to clinical teams and patients
Identify and drive performance improvement efforts focused on patient identification and enrollment practices (in partnership with the local medical center team)
A clinical background is not required, and candidates should be comfortable working in a patient care setting where they will work directly with patients, front-line physicians, nurses, PCCs, and other members of the ED and Hospital care team

Job Summary:
In addition to the responsibilities listed below, this position is also responsible for developing health education materials and providing training on health education and new concepts, processes, and programs related to health behavior change in the health education departments to new and current health education employees; documenting the success and comprehension of training materials while maintaining updates of central training repository; teaching health behavior change practices, policies, and procedures for new and existing staff; creating and organizing the learning objectives, course outlines, assessments, training plans, facilitator guides, participant workbooks, handouts, review activities and audio-visual aids for all health education workshops; and facilitating and consensus building with subject matter experts to develop health education materials for various delivery mediums, including, but not limited to print, digital, web, video, electronic medical record
