Job Title: Social Care Network Nutritionist/Service Coordinator

Location: TOUCH, Congers NY. Hybrid position including remote work.

Reports to: TOUCH Social Care Network Administrators

Full-time position with an option for part-time work for qualified candidates.

TOUCH provides nutritional counseling as we distribute food for more than a million meals per year to our neighbors and families in need.

Job Description: Social Care Network Nutritionist/Health Educator

We seek a compassionate and dedicated nutrition educator to help screen and enroll eligible people for services as part of the Hudson Valley Social Care Network (HVSCN). The HVSCN is providing these services under a Section 1115 Medicaid waiver enabling the use of Medicaid funds to meet people’s health-related social needs and improve their health and well-being. TOUCH’s Social Care Network nutritionist/health educator will play a key role in supporting the health and well-being of our clients.

Essential Job Responsibilities include:

1. Completing introductory Social Care Network trainings

2. Screening and making referrals for appropriate health related social needs (HRSNs) (e.g., nutrition, housing, transportation)

3. Assessing dietary needs and developing individualized care plans

4. Providing nutritional counseling and health education for eligible clients

5. Accurate and timely documentation of services

6. Conducting periodic reassessments of service needs

7. Organizing and/or conducting nutrition education groups in person or online

8. Performing additional duties as assigned

Requirements

College degree in human services (e.g., nutrition, social work, public health) or equivalent experience and education.

Excellent interpersonal communications skills, Spanish or Haitian Creole language skills a plus

Commitment to TOUCH’s mission and to work in a positive and collaborative workplace

Must be comfortable and proficient with online documentation of services

Familiarity with the Hudson Valley community and its resources is a plus.

Commitment to compliance with HIPAA privacy and confidentiality rules
