Job Description
Summary
Collect and assess data relevant to individual psychosocial needs. This includes individuals with mental, emotional, or substance abuse problems, including abuse of alcohol, tobacco, and/or other drugs. Activities may include individual and group counseling, crisis intervention, case management, client advocacy, prevention, and education. Responsible for developing a care plan that best suits a patient individual needs. Person must be able to use good judgement and experiences when developing individualized care plans.
Education
A Master’s or Bachelor’s degree in psychology, mental health, social work or sociology from an accredited institute and/or related majors may be acceptable.
Experience
Two to three years’ relevant or direct experience within social services preferred. Extensive and effective communication skills, reporting analysis, knowledge and experience are needed for this occupation. Person must have ability to accurately document patient observations.
Licenses, Certification and/or Registrations
Social Worker licensure for the state of Alabama and/or with at least two years of supervised clinical experience.

Duties & Responsibilities
Develop care plans that include counseling, support services, treatment methods and referrals.
Explain care plans goals and objectives to patients and their families.
Periodically review patient and patient family situations and make changes to the care plan as necessary.
Maintain patient records, preparing reports, monitoring progress toward treatment goals.
Conducting annual review of active treatment/care plans.
Offer individual and group counseling sessions to patients.
Provide crisis interventions, arrange for services from referral agencies and help patients transition back into the community.
Provide follow-up and aftercare planning as needed.
Develop, implement and maintain partnerships with other community providers.
Attend staff meetings, supervisory conferences, and other activities, which ensure the smooth functioning of clinical operations. Perform other duties as assigned.
Contact RHMPI patients to assess any psychosocial needs/barriers related to COVID-19 pandemic.
Link / refer patients to local or federal resources to address needs or barriers related to COVID-19.
Provide education and support regarding COVID-19 testing and vaccines.
Language Skills
Ability to read, speak effectively, analyze and interpret documents in English and/or Bilingual Spanish.
Specialized Skills
The person must be self-motivated, able to manage conflicting priorities, and be results oriented. Possess superior organization and time management skills and ability to engage appropriately directly and indirectly with peers and the medical team.
Computer Skills
Have a working knowledge of the following software programs: Microsoft Word, Outlook, Excel, PowerPoint and Publisher.
