The Physical Therapist provides physical therapy evaluation and treatment to patients with musculoskeletal and/or neurological impairments. Also responsible for the monitoring and documentation of patient status, treatment and response.
Responsibilities of this position include:
Performs physical therapy evaluations and develops a plan of care that is appropriate for the diagnosis and age of patient.
Performs physical therapy patient treatment as per the plan of care and modifies treatment plan based on ongoing assessment of patient condition.
Demonstrates an understanding of the operation and indications for commonly used modalities and equipment.
Documents evaluations, daily notes, progress notes, and discharge notes in a timely manner; therapy charges are supported by this documentation.
Communicates and documents necessary information appropriately with patients, families, physicians, and interdisciplinary team members.
Performs other duties as assigned.
Completed department-specific orientation and annual competencies for this position. Requirements for this position include:
Bachelors Degree required.
Masters Degree preferred.
Works well under pressure preferred.
BLS certification, valid Wisconsin driver's license, license to practice physical therapy in Wisconsin required. Benefits:
