Position Title:
Transition Pathways Nurse
Assignment:
Flexible hourly schedule as needed,
Up to 288 hours per school year (the equivalent to 1 day/week)
Reports to:
Transition Pathways Supervisor and Director of Special Services
Qualifications:
Bachelor of Science in Nursing
Illinois Nursing License
RN, Professional Educator License with School Nurse Endorsement (PEL-SN) preferred but not required Calendar:
School Calendar & Summer School (Special Education ESY)

Responsibilities:
Collaborate in the development of individual student health care and emergency care plans for students with special care needs for the purpose of providing direction to site personnel and/or implementing directions provided by parents and/or physicians
Consult with student's educational team to ensure that the health care plan can be implemented in program, community and vocational settings
Collaborate with the Pathways Supervisor to ensure that medical equipment and supplies are ordered and stocked as necessary and function appropriately
Demonstrate cooperation and collaboration when contributing to the educational team
Collaborate in the design and use of documentation processes with the educational team and Pathways Supervisor to ensure that pertinent health information and necessary accommodations are afforded to meet the medical needs of students
Complete medical reviews or health history reports as part of the IEP process (with CSN supervision if necessary)
Participate in educational team reviews to assess situations involving students' health related issues for the purpose of identifying problems, referring for proper treatment and complying with legal requirements
Collaborate with the Pathways Supervisor in the administration of first aid and/or medication to staff / students
Collaborate with parents, students, teachers, staff, health care providers, and/or public agencies for the purpose of promoting and/or securing student health services, providing health & wellness information (i.e., mental health wellness, healthy lifestyles, substance abuse, personal safety, nutrition, food allergies, independent medical and mental health decision-making, self-advocacy related to medical needs, etc.) and for compliance with legal requirements
Maintain students' confidential files and records (e.g. health care plans, agency referrals, accident reports, immunization records, etc.) for the purpose of providing information required by legal requirements and professional standards
Report health and safety issues to assigned administrator and appropriate agencies for the purpose of maintaining students' personal safety, a positive learning environment and complying with regulatory requirements and established guidelines
Perform other related duties as assigned for the purpose of ensuring the efficient and effective functioning of the extended transition services setting

$39.14
