Job Description
Coordinates team-based care to provide health services to individuals, through active partnerships with patients, caregivers, families, and other health professionals. Facilitates a "shared goal model" within and across settings to achieve coordinated high-quality care that is patient and family centered.
Job Responsibilities:
Assesses patient, recognizing diagnoses of problems and/or anomalies
Education of Patient
Plan and implementation of appropriate care
Provides complete and accurate documentation of each patient contact
In the event of staffing changes, patient events/circumstances, accepts and performs subordinate role and alternate assignments within scope of practice to meet the needs of patient
Other duties as assigned
Job Requirements: Must be currently licensed by the State Board of Nursing and maintain current protocols as a Nurse Practitioner as outlined by the State Board of Nursing. Master's degree in nursing with certification as Nurse Practitioner required. Experience strongly preferred.
