Behavioral health agency located in Washington DC has an exciting opportunity for Nurse Practitioners, Physician Assistants, Licensed Practical Nurses and Registered Nurses to join our team. Description: Provide clinical services ranging from documenting personal histories to triage and Medication/Somatic Treatment.
Check consumers vital signs and height and weight such as blood pressure and body temperature
Clinical assessment of clients, including taking vital signs, recognizing critical consumers and providing consumer education.
Discuss care with consumers, listening to their concerns, and relaying concerns to the psychiatric team.
Keep up-to-date medical records
Scheduling appointments, complete screenings and assessments
Provide information and referrals to community resources
Follow up on referrals to determine the case outcome (when appropriate) and/or document the needs for the development of community services
Provide limited case management services on psychiatric referrals for social services and related cases
Attend meetings, communicate pertinent information to Psychiatrist and other administrators
Identify clinical, outreach and administrative needs and challenges of the Psychiatry Department
Report barriers for treatment success
Document all billable and non-billable services in Credible EHR
Uphold ethical standards and responsibilities
Report all incidents to QI
Advocate for clients and client needs (internal and external)
Maintain quality and timely documentation
Act as an Ambassador of One Care in the community
Contact pharmacies regarding current prescriptions, medication reconciliation and arrange the delivery of medications, including long acting injectable medications.
Maintain the storage of long-acting injectables and give injections, as needed.
Contact Primary Care Physician for annual physical exam and results of lab tests
Assist with clients consent for Meds
Copy prescriptions, labs orders and file them into consumer files
Coordinate psychosocial needs of consumers with therapist
Required Qualifications:
Must be a licensed LPN or PA in the District of Columbia.
Experience with acute care, co-occurring disorders in a mental health and/or emergency treatment setting.
Shall have basic office equipment (copier/fax) and computer literacy skills.
Experienced with data or word processing applications (Microsoft Office Suite Excel, Access, Word, etc.) and working in an electronic health record management system
Experienced in evaluating the effectiveness of the nursing care plans.
Be organized, effectively manage competing deadlines, have excellent written and verbal communication skills
