RESPONSIBILITIES
Patient Care & Service. Assist residents with activities according to their individualized care plans, support transportation to and from activities, facilitate leisure activities, conduct evaluations, and provide emotional support to residents and their families.
Safety. Maintain a clean, safe, and nurturing environment for residents.
Communication & Documentation. Support family members, direct them to appropriate professional staff, respond promptly to resident requests, and exchange reports with team members and leaders.

CNA - Certified Nursing Assistant - New York State Education Department (NYSED)
PHYSICAL REQUIREMENTS:
M - Medium Work - May require exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing, or squatting.
