Case Worker (Health Programs)
Reports to: Director of Community Engagement and Outreach
SUMMARY:
The Case Worker will assist the Delta HOME grant in establishing a network of care between 6 county service area and program participants in need of home modification.
MINIMUM QUALIFICATIONS:
• Licensed Social Worker, master's preferred
• 2 years of experience providing guidance to clients involving conducting home visits, community resources, case or care management
• Excellent communications skills
ESSENTIAL DUTIES:
• Collaborate with program staff, clinics, hospitals with primary responsibilities related to family dynamics, relationships, and adjustment problems. Act as a liaison for community referrals and resources identification.
• Maintain current knowledge of DHA and other resource networks for planning and provisions to meet Social Services needs in the target area.
• Enroll participants in Home for Life Design program, ETO and maintain ETO.
• Ensure accurate data entry for caseload.
• Travel with Occupational Therapist Assistant to assist with home visits.
• Assist in developing a case management system while ensuring the maintenance of ETO.
• Coordinate the available resources to serve families, including follow-up and referral services.
• Other duties as assigned.
