Occupational Therapist - Occupational Therapy

PRN (as needed)

Occupational Therapist is responsible for providing Occupational Therapy Services, inclusive of performing examination and evaluation, determining a diagnosis and developing individualized plan of care and plan for discharge. Determine functional diagnosis by utilizing physician referral, medical records and occupational therapy evaluation. Establish goals to maintain, improve or restore functioning, alleviate pain and prevent physical dysfunction in patients.

QUALIFICATIONS

Education:

• Bachelor of Science degree in Occupational Therapy from an accredited school of Occupational Therapy.

Licensure:

• Occupational Therapy licensed issued by the state of North Carolina.

Necessary Skills:

• Maintains strictest confidentially; adheres to all HIPPA guidelines/regulations

• Must be well organized and detail-oriented.

• Possess good communication skills

Functions of the Position:

• Occupational therapy evaluation / treatments will be rendered only with a physician referral.

• Provide occupational therapy treatments; supervise treatments provided by certified occupational therapy assistants and revise treatments as needed.

• Recommend and assist in providing residents with functional equipment (wheelchairs, adaptive equipment, etc.).

• Work closely with physician, other professionals and consultants to integrate occupational therapy into the multi-disciplinary team care plan.

• Participate in resident and family education and meetings, including team conferences, and discharge planning.

• Document patient / resident admission and discharge evaluations per occupational therapy department policy.

• Document progress notes to update status of those patients/ residents receiving occupational therapy.

• Document readmissions, re-evaluations, consultations and other specific requests per department policy.

• Participates in seminars, conferences, and activities for professional development.
