Lutheran Social Services of Central Ohio is currently seeking a Service Coordinator in Columbus, Ohio.

What will I do as the Service Coordinator with Lutheran Social Services?
Provide general service management including intake, education, documentation and formation of informal support networks and referral of residents to service providers in the general community in relation to issues including but not limited to counseling, Meals-on-Wheels, financial assistance, home health aides, preventative health screening, homemakers and transportation.
Monitor the ongoing provision of services from community agencies and keep case management plans current for residents within assigned HUD facility as required by federal, state, local HUD and LSS regulations and manage the provision of supportive services to residents in assigned HUD facility where appropriate.
Investigate, develop and execute educational events relating to health care, agency support, life skills and referral sources and develop and distribute resource directories for assigned HUD facility that include listings of state and/or local service providers in relation to issues including but not limited to counseling, Meals-on-Wheels, financial assistance, home health aides, preventative health screening, homemakers and transportation.
Serve as liaison to general community, community agencies, network with community providers in order to promote assigned facility and seek out new services available to the residents of assigned HUD facility.
Provide reports and updates to Community Manager regarding resident needs and progress, not including confidential resident issues.
Perform other duties as assigned. Requirements for the Service Coordinator with Lutheran Social Services:
Associate's degree in social work, psychology, counseling, gerontology, or related field. Bachelor's degree preferred.
Two to three years experience in social service delivery and case management or equivalent level of education and experience.
Valid Ohio driver's license, access to reliable insured transportation and ability to meet LSS insurance standards required As one of the largest non-for-profits in Central Ohio, we value our employees.
