Manageable Caseloads: Care for 56 patients per day within a localized service areano more 8+ visits across multiple counties

Mileage Reimbursement: Generous rate of $0.625 per mile

Key Responsibilities

Provide physical therapy services to patients under the direction and supervision of a licensed Physical Therapist

Assist with implementing individualized care plans to help patients restore function, improve mobility, and maintain independence at home

Collaborate with patients, families, and interdisciplinary care teams to ensure high-quality, patient-centered care

Accurately document treatments, progress notes, and communications in compliance with regulatory and agency guidelines

Contribute to case conferences and team meetings to ensure effective care coordination

Qualifications

Active Physical Therapist Assistant (PTA) license in the State of Ohio (required)

Minimum of 1 year of recent experience as a PTA in a healthcare setting (hospital, SNF, ALF, hospice, or home health)

Home health experience preferred, but willing to train the right candidate

Strong communication and interpersonal skills

Highly organized, self-motivated, and able to work independently

Reliable transportation and ability to travel within a local territory
