Position Title: Outreach and Triage Coordinator, Massachusetts Veteran Connect

Department: Human Services Department

Exemption: Non-Exempt

Supervisor: Director of Massachusetts Veteran Connect

Supervises: MVC Specialist(s)

Job Summary:

The Massachusetts Veteran Connect Program (MVC) is funded by the Massachusetts Executive Office of Veterans Services to provide outreach, case management, homelessness prevention and stabilization services and financial assistance to underserved Veterans across Eastern Massachusetts. This program targets Veterans impacted by poverty, racial inequities, domestic violence, behavioral health diagnosis, acute medical conditions and/or other disabilities that are barriers to self-sufficiency. Additionally, this program aims to identify and connect with Veterans who may not be eligible for U.S. Department of Veterans Affairs funded programming to fill a gap in Massachusetts Veterans Services by providing a parity of services for all Veterans who may be at risk or currently experiencing homelessness.

Job Responsibilities

Develop and coordinate evidence-driven outreach plan with Director of MVC. Work alongside MVC Specialist and SSVF Outreach teams to implement this plan and identify Veterans for supportive services through street, community-based, and shelter outreach.

Provide regular supervision and training to MVC Specialist(s).

Manage MVC Airtable referrals by conducting screening calls of the Veterans referred to MVC for supportive services. Review information with Director of MVC at weekly in-person supervision meetings.

Follow up with referral sources to fill any gaps in knowledge and request additional information if-needed.

Ensure Veterans are quickly linked to financial assistance, housing path and/or other resources that will help resolve and/or prevent their housing crisis.

Identify any immediate documentation needs of Veterans that are required for engaging with and soliciting requests community-based, housing, and government entities (DD214, income documents, MA ID, etc.).

Assist Veteran households and identify an immediate safe place to stay within their own network if required; provide follow up services to ensure Veterans are linked to proper NECHV program (MVC or SSVF).

Triage referrals to prioritize Veterans in acute situations and/or make referrals to other organizations if Veteran is unable to utilize MVC services.

Provide MVC Specialists with detailed contextual information regarding Veteran’s current

situation to promote efficient uptake of service need identification and engagement.

Use Motivational Interviewing, Harm Reduction, Trauma Informed Care and Progressive Engagement strategies to develop appropriate therapeutic rapport.

Educate community agencies and collaborating organizations about MVC and New England Center and Home for Veterans to identify Veterans who are experiencing or at risk for homelessness.

Engage in direct street outreach to Veterans experiencing homelessness across Eastern Massachusetts.

Maintain appropriate documentation, including completing intakes, documenting outreach attempts to agencies and case notes in an electronic database.

Provide training and mentorship to team of MVC Specialists to help them navigate complex landscape of social services (Veteran and non-Veteran).

Work with the Outreach teams to conduct outreach to promote Center activities and groups; lead and participate in groups as needed. Attend Program, Department meetings and trainings as well as Continuum of Care provider meetings and other meetings with collaborating agencies as required.
