Job Description
Position Summary:
We are seeking a compassionate, motivated, and experienced Substance Use Therapist Case Manager to provide therapeutic services and case management to individuals struggling with substance use disorders. The ideal candidate will use evidence-based practices to support recovery, develop treatment plans, and coordinate supportive services while working collaboratively with a multidisciplinary team.
Key Responsibilities:
Conduct comprehensive biopsychosocial assessments and intake evaluations
Develop and implement individualized treatment plans in collaboration with clients
Provide individual, group, and family counseling using evidence-based approaches (e.g., CBT, MI, 12-step facilitation)
Maintain accurate and timely documentation in accordance with HIPAA and agency guidelines
Coordinate case management services including referrals for housing, employment, mental health, medical care, and legal services
Monitor clients’ progress and adjust treatment goals as needed
Facilitate relapse prevention planning and crisis intervention
Collaborate with internal and external providers (probation officers, social workers, medical personnel, etc.)
Participate in regular team meetings, supervision, and training
Maintain professional licensure and stay updated on trends in substance use treatment
Qualifications:
Required:
Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or related field
Certification or licensure as an addiction counselor (e.g., CADC, LADC, LCAS, or equivalent)
Minimum 1–2 years of experience in substance abuse treatment or behavioral health
Strong understanding of addiction, co-occurring disorders, and recovery models
Excellent communication, documentation, and organizational skills
Ability to work with diverse populations with sensitivity and cultural competence
