Responsibilities
As a Social Worker, you will collaborate closely with psychiatric medical providers and therapists on the patient's primary treatment team to ensure a cohesive and effective plan of care.
Hours for this role are Monday - Friday, 8a-430p
Responsibilities and Accountabilities:
Assessment and Planning: Conduct effective assessments and suicide risk evaluations, develop treatment and discharge plans, and lead educational or therapeutic groups.
Patient and Family Communication: Maintain open lines of communication with patients and their families to ensure they are actively involved in treatment decisions.
Care Coordination: Coordinate care with outpatient providers during patient admissions and discharges to ensure continuity of care.
Team Facilitation: Facilitate weekly treatment team meetings, creating and updating master treatment plans based on team consensus.
Clinical Contribution: Bring a unique clinical perspective to the interdisciplinary team, drawing on training in family systems and social work theory
Qualifications
Master's Degree in Social Work preferred, with licensure in the State of Minnesota required.
Experience in healthcare setting preferred
Experience using electronic health records preferred
Proficient in Microsoft Office Products including Word and Outlook
Able to effectively and efficiently communicate in written and verbal form
Ability to smoothly and rapidly shift between dissimilar tasks
Ability to communicate well both verbally and in writing
Ability to react quickly and calmly as part of a team to handle volatile and potentially emergency situations
Ability to make effective decisions while in stressful situations and circumstances
After training, be able to manage disruptive and explosive patients.
Compensation for this role ranges from $26-34 per hour depending on education and experience.
