In Demand The Care Manager is responsible for providing quality professional care management services to all clients and their responsible parties. Our objective is to assist our clients in managing and navigating challenges in aging as well as Adults with physical and mental disabilities and providing the highest quality of life. This includes:
Biopsychosocial Assessment
Care Coordination in home, hospital, rehab, senior community
Life care management
Medical oversight and navigation
Interfacing with medical personnel
Advocacy, information and referrals Qualifications:
Proficiency in assessments of all care management domains resulting in development of personalized care plan.
Communication skills: clear written and verbal communication with clients, families and healthcare team is essential to coordinating care.
Strong organizational skills in order to manage multiple clients, appointments and care plans
Ability to quickly identify issues and implementing effective solutions is critical to effective outcomes.
Knowledge of healthcare systems; insurance, community resources to help clients and families navigate care needs
Recognizes industry trends and problem solves
Respectful of company and client confidentiality.
Ability to ensure inappropriate placements, duplication of services, and unnecessary hospitalizations are avoided.
Manage time efficiently
Sensitivity and understanding of diverse cultural backgrounds to provide appropriate client centered solutions
Microsoft 365 skills and additional care management application. Compensation: $45.00 - $50.00 per hour
