Nurse Practitioner or Physician Assistant

A well-established Family Practice group is seeking a full-time Nurse Practitioner or Physician Assistant to join our dynamic team. This position includes caring for patients in both office and nursing home settings across our Palm Coast and St. Augustine locations.

We are looking for a skilled and compassionate provider with experience in primary care and nursing home environments. The ideal candidate will feel confident managing patients independently while also working collaboratively within a supportive care team.

Qualifications:

Active Florida NP or PA license required

Strongly preferred: experience in family medicine and long-term care settings

Ability to work autonomously and as part of a team

Excellent clinical judgment and communication skills
