Job Summary:

OMC, a rural hospital dedicated to providing high-quality healthcare services to underserved communities, is seeking a compassionate and dedicated Advanced Practice Clinician (APC) with Family Medicine experience to join our team. This full-time position offers an opportunity to work in a close-knit, supportive environment while making a significant impact on the health and wellbeing of our rural population. Key Responsibilities:

Provide comprehensive, patient-centered care to a diverse patient population, including infants, children, adults, and elderly patients.

Diagnose, treat, and manage acute and chronic medical conditions, both independently and in collaboration with physicians and healthcare teams.

Perform routine physical exams, wellness check-ups, immunizations, and preventive screenings.

Manage and follow-up on patients with chronic conditions such as diabetes, hypertension, asthma, and heart disease.

Provide health education and counseling to patients and their families on disease prevention, healthy lifestyle choices, and overall wellness.

Collaborate with the medical team, including physicians, nurses, and specialists, to ensure comprehensive care and treatment.

Be part of the on-call rotation to provide after-hours and weekend coverage for inpatient and emergency department care.

Maintain accurate and up-to-date patient records using the hospital’s electronic health record (EHR) system.

Adhere to hospital policies, procedures, and ethical standards.

Qualifications:

Nurse Practitioner (NP) or Physician Assistant program (PA).

Minimum 1-2 years of experience in patient care as an APC

Valid, unrestricted license to practice as an APC in Wisconsin.

Certification in Family Medicine or relevant experience in primary care.

Strong clinical skills, with the ability to diagnose and manage a wide range of medical conditions.

Excellent communication and patient care skills, with the ability to build trust and rapport with patients.

Ability to work collaboratively in a multi-disciplinary team setting.

Compassionate and patient-focused, with a commitment to delivering high-quality care.

Flexibility and adaptability in working within a rural healthcare environment.

Preferred Skills:

Experience with rural healthcare challenges and resource limitations.

Proficiency with electronic health records (EHR) systems. EPIC experience a plus.

Working Conditions:

Full-time position as a .8 FTE.

Every other Saturday
