Community Engagement LPN (Part-time)

Position Summary

LPNs at KCHC support physicians, nurses, and medical assistants in delivering high-quality, patient-centered care. They see patients within their scope of practice, assist with complex cases, and help execute care plans. LPNs are flexible team players who adapt quickly to change, make confident decisions, and work to ensure that patient care is both timely and effective. This role includes administrative, clinical, and patient education duties and contributes to a coordinated, team-based healthcare model.

Essential Duties and Responsibilities

Clinical Support & Patient Care

Support daily clinic functions based on team needs.

Conduct intake appointments including medical histories and agenda-setting for provider visits.

Assist with Annual Wellness Exams, screenings, history taking, medication reconciliation, and identification of care gaps.

Provide patient education and follow-up care within LPN scope.

Perform basic clinical measurements.

Collect and prepare lab specimens; sterilize instruments and ensure proper disposal of contaminated supplies.

Maintain up-to-date BLS certification and participate in emergency response activities.

Respond to patient phone calls, provide triage support, and help facilitate timely access to care.

Work independently within scope using sound clinical judgment while adhering to policies and procedures.

Care Coordination & Team-Based Care

Support care coordination efforts in collaboration with Health Educators, clinical staff, and external partners.

Participate in daily huddles and ongoing care coordination efforts.

Work closely with NC Field, Parent-as-Teacher, Prevent Blindness, American Heart Association, Lenoir County Migrant Education Program, and other community partners to ensure program compliance.

Documentation & Administrative Support

Accurately document patient interactions, services, and progress in the Electronic Health Record (EHR).

Assist with administrative duties such as data entry, documentation, and reporting.

Compliance & Safety

Maintain OSHA and HIPAA compliance across all clinical areas.

Comply with internal policies and procedures, including adherence to the KCHC Employee Handbook, dress code, and behavioral expectations.

Participate in required training related to HIPAA, clinical hygiene, and other regulatory topics.

Support organizational performance improvement and corporate compliance programs.

Community Engagement & Representation

Attend and support community events, including setup and breakdown of materials.

Participate in community meetings and represent KCHC as needed.

Maintain current knowledge of available community resources and refer patients appropriately.

Professional Development & Team Engagement

Attend staff functions and participate in professional development opportunities.

Collaborate and communicate effectively as part of a multidisciplinary team.

Other Duties

Perform additional duties as assigned.

Qualifications

Education:

Graduate of an accredited practical nursing program

Certifications & Licenses:

Valid LPN license in the state of North Carolina.

Experience:

2–5 years of nursing experience preferred.

Skills:

Proficiency with EHR systems and Microsoft Office Suite.

Strong communication and organizational skills.

Competence in patient education and chronic disease management.

Work Environment

Must be able and comfortable working in a variety of settings including, but not limited to, clinical environments, office spaces, and outdoor locations.

This role may involve exposure to low, normal, and high temperatures, as well as varying weather conditions during outreach or event-based activities.

Must be able to work nights and weekends as departmental needs arise.

Travel Requirements

Regular local travel is required using a company vehicle.

Employee must adhere to all vehicle safety policies and participate in routine vehicle inspections and in-service trainings.

Physical Demands

Occasionally required to sit, walk, reach, and handle materials.

Supervisory Responsibilities

Provides indirect supervision by mentoring and training staff as needed to support departmental goals and service quality.
