Position Overview:

If you possess a passion for caring for the elderly and mentoring care assistants, then this position is perfect for you! This is a fantastic opportunity to become an integral part of our growing team! We are looking for a passionate and dedicated Lead to help supervise our PM shift.

Job Responsibilities:

Serve as the main point of contact to the Clinical Care Assistants when they have questions

Serve as the liaison between the Facility and the Manager on Duty (MOD) when working

Provide daily operational checks and balances to CCAs to validate their daily checklists are fully and accurately completed

Serve as the main point of contact for external parties who may have questions or concerns, i.e. Hospice, Home Health, Powers of Attorney, families, etc.

Seek appropriate staffing replacements when working to ensure adequate coverage is available to meet resident care needs timely

Serve as MOD Friday 4:00pm Monday 8:00am as part of the larger MOD rotation, to be completed during the course of a normally scheduled working weekend.

Obtaining specimen samples as directed by the Director of Nursing or designee upon order from physician.

Performing daily rounds to monitor and evaluate resident care, proper medication storage area, equipment, etc.

Maintaining professional boundaries, to not contribute or spread gossip, and to cease observed instances from CCAs.

Caring for residents in the community by adhering to all community policies and procedures.

Reviewing all tasks before performing cares to ensure updated cares are being completed.

Assisting residents with Activities of Daily Living (ADL's) to include but not limited to; bathing, showering, dressing, grooming, shaving, nail care, hair care, eating, making bed, doing laundry, ambulating, and helping to activities and other functions.

Answering telephones and assisting visitors in an assertively friendly manner.

Inputting residents on the correct spa bath/shower day according to the Bath/Shower Scheduling Reference Guide and updating as needed, communicating all changes through the Director of Nursing or designee.

Completing a spa bath or shower on residents that have refused or missed previously during the week, to ensure all residents have been properly bathed.

Providing personal care for residents according to the Individual Service Plan (ISP) with a respectful attitude and without rushing them.

Providing step-by-step instructions to residents before and during cares.

Completing skin checks and weights as deemed necessary per ISP, physician orders, or SHO, and reporting any concerns to the RN/LPN or designee.

Promoting privacy and confidentiality all while maintaining resident self-esteem in a comfortable physical environment.

Demonstrating infection control procedure by handling and disposing of linens, soiled clothing, or supplies properly.

Providing support for the facilitation of communication between Clinical Care staff and Management Team to ensure all communication is received/delivered by/to the Clinical Care Assistants and Management Team.

Qualifications:

A valid Medical Assistant Certification preferred, but not required.

Minimum of 3 years' experience in an assisted living or healthcare setting

Additional education in rehabilitative or geriatric nursing preferred, but not required.

Minimum of 1year of managerial experience required, preferably in a healthcare setting

CBRF Registry, CPR/AED certified, and Alzheimers/Dementia care experience preferred, but not required.
