The Admissions RN is a registered nurse and key team member responsible for presenting hospice services and philosophy to patients and families to facilitate appropriate admissions.
Participates in information gathering, analysis, education, and documentation during the admission process.
Reviews medical records to assess hospice suitability and obtains agreement for admission from eligible patients and families.
Collaborates with the LPN to analyze data and secure admission orders.
Builds strong referral relationships to support high-quality, responsive care.
Maintains clinical competence and develops internal relationships with care teams.
Uses interpersonal skills to educate and address questions about hospice care.
Qualifications include RN licensure, healthcare experience, strong communication, and clinical knowledge; a bachelor’s degree is preferred.
