Medical Social Worker Home Health

The Medical Social Worker (MSW) provides psychosocial support to patients and families in home health settings, addressing emotional, psychological, financial, and environmental needs to promote optimal health outcomes.

Conducts assessments to identify social and emotional needs.

Develops personalized care plans with the interdisciplinary team.

Connects clients to community resources like financial aid, housing, transportation, and support groups.

Assists with healthcare legal documents such as advance directives.

Advocates for patient needs within the healthcare system.

Maintains accurate documentation in compliance with regulations.

Participates in case conferences and coordinates care with healthcare professionals. The role requires a Master’s in Social Work (MSW), Florida LMSW license, knowledge of community resources, strong communication skills, ability to work independently, and reliable transportation. Compensation ranges from $40.00 to $60.00 per hour, with 8-24 hours weekly, primarily day shifts in home health settings.
