Description

New and unique opportunity to meet with patients who arrive at the emergency department (ED) with primary SUD or mental health concerns. The interventionist will be embedded into the ED and will attend rounds with the ED medical team to assess patients’ behavioral health needs and develop a discharge plan for linkage to behavioral health treatment and other resources as needed, including follow up appointments. The interventionist will work within the Ohio Health System of Care in Franklin County, primarily at Grant Hospital.

Responsibilities:

Engage with patients in the ED with a behavioral health diagnosis.

Perform outreach and consultation to intervene with individuals, family members and significant others and provide linkage to resources for SUD and mental health services.

Conduct rounds with the medical team in the ED.

Use the Stages of Changes and Motivational Interviewing to support decisions regarding support and treatment.

Seek consultation with the medical team to provide appropriate intervention based on diagnosis on an individual basis.

Provide consultation and educational information on substance use disorders, mental health and social determinant of health services to patients, families and community organizations.

Provide case management responsibilities with patients as needed.

Qualifications

Minimum of AA/BA required; Chemical dependency license or SUD in scope required.

Prior experience with SPMI and/or SUD population required.

Strong interpersonal and communication skills.

Ability to work effectively as part of an interdisciplinary team.

Crisis intervention and de-escalation skills.

Knowledge of community resources and healthcare services.

Valid driver's license and insurance.
