Blueprint Program Manager

Gifford Health Care is seeking a Blueprint Program Manager to oversee the programmatic and administrative components of our Blueprint for Health grant agreement. This individual will serve as the local contact for the program, working closely with the State and actively participating in statewide Blueprint meetings and initiatives.

Responsibilities include managing the financial budget of the Blueprint Grant for our Health Service Area (HSA), coordinating required reporting for evaluations and payment, and ensuring active participation from area primary care practices. The Program Manager will support these practices in achieving Patient-Centered Medical Home (PCMH) recognition by providing facilitation, IT reporting support, and connecting them with Community Health Team (CHT) resources. Additionally, the role involves leading local recruitment efforts for provider participation, collaborating with State-hired facilitators, and contributing to quality improvement initiatives. Strong leadership skills and knowledge of healthcare reform, clinical environments, or nonprofit work are preferred.
