As aNurse Practitioner,you will:

Conductcomprehensive in-home Health Risk Assessments (HRAs)for Medicare Advantage members.

Reviewmedical history, medications, lifestyle, and social determinants of health.

Performphysical and mental health evaluationsand document findings in real-time.

Providepreventive care educationwithout the need for ongoing patient management.

Travel locallywithin your county and surrounding areas for in-home assessments.Qualifications

ActiveNurse Practitioner (NP) (all specialties except PMHNP)

Minimum of 6 full 8-hour days per month (flexible scheduling available).

Must be willing to travel to all counties listed below + open to other counties in the area and accept a 3-month contract.
