Position Summary:

Pomerene Family Care is seeking a highly motivated and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our family medicine team. The ideal candidate will provide comprehensive, high-quality primary care to patients of all ages in a collaborative and patient-centered environment. This position plays a critical role in delivering preventative care, chronic disease management, and acute illness treatment within a rural community-focused health system.Key Responsibilities:

Perform thorough physical examinations and health assessments for patients across the lifespan.

Diagnose and manage acute and chronic medical conditions in collaboration with supervising physicians as needed.

Order and interpret diagnostic tests including labs, imaging, and other studies.

Develop and implement patient-centered treatment plans, with a strong emphasis on education and preventative care.

Prescribe medications and treatments according to state and federal regulations.

Provide health counseling and education to patients and families regarding wellness, disease prevention, and healthy lifestyle choices.

Maintain accurate, timely, and complete documentation in the electronic medical record (EMR).

Participate in quality improvement initiatives and contribute to ongoing development of clinical protocols and standards of care.

Collaborate effectively with physicians, nurses, support staff, and specialists to ensure comprehensive care.

May assist in precepting students or participating in community health events.Qualifications:

Licensed as a Nurse Practitioner (NP) or Physician Assistant (PA) in the State of Ohio.

Minimum of 2 years' experience in family practice or primary care preferred; new graduates with strong clinical backgrounds will be considered.

Current DEA license and prescriptive authority in Ohio, or ability to obtain

BLS certification required; ACLS preferred.

Strong interpersonal and communication skills, with a commitment to compassionate, patient-centered care.

Ability to work both independently and as part of a team in a fast-paced outpatient environment.Working Conditions:

Outpatient clinical setting, Monday–Friday

Minimal shared call required
