About The Position

Nurse Practitioner - Primary Care Clinic

Join our Akron healthcare team as a dedicated Nurse Practitioner, providing exceptional, value-based primary care to Medicare patients. This permanent position in our clinic offers a collaborative environment focused on quality care and building meaningful patient relationships.

Job Details

Employment Type: Permanent

Schedule: Monday - Friday, 8 AM to 5 PM

Location: On-Site in Akron, OH

Job Requirements

Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate

National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC

Active, non-probationary state Nurse Practitioner license

Active DEA license

US Work Authorization

Minimum of 1 year of experience in primary care: internal medicine, geriatrics, or family medicine preferred

Responsibilities

Provide exceptional primary care to Medicare patients

Conduct office visits for routine and acute issues

Administer annual wellness visits and health risk assessments

Coordinate care with other healthcare providers, specialists, and agencies

Lead population health initiatives in coordination with the Care Team

Assist with phone triage and outreach

Educate patients on health conditions, care plans, and treatments

Participate in promotional activities and conduct home visits as needed
