St. Croix Health is looking for a casual patient focused, Hospital Pharmacist to join our team.
The Pharmacist is responsible for ensuring the safe and effective use of medications, providing high-quality pharmaceutical care, and promoting wellness to patients. The pharmacist works closely with other healthcare professionals to support optimal patient outcomes and contribute to the health and well-being of our community.
1. Prescription Preparedness
Review medication orders for diagnosis, allergies, duplications, and interactions.
Dispense medications: Accurately prepare and dispense prescription medications according to legal, regulatory and organizational standards.
Patient consultation: Provide advice and information to patients regarding proper medication use, side effects, potential interactions and the importance of adherence to prescribed regimens.
Medication management: Review patient prescriptions and medication profiles to ensure safe and effective medication therapy. Identify and resolve potential medication-related problems
Medication error review: Monitor for medication errors and adverse drug reactions and document accordingly.
Compounding: Prepare customized medications for patients when necessary, follow the specific instructions and requirements provided by prescribers and organization standards.
Collaboration with healthcare team: Work closely with physicians, nurses, and other healthcare providers to optimize patient care plans and medication therapy management.
Inventory management: Monitor and manage medication inventory to ensure adequate stock and proper storage conditions. Order medications and supplies as needed.
Patient education: Educate patients about managing chronic conditions, lifestyle changes, and preventative health strategies. Provide vaccination services where applicable.
Regulatory compliance: Ensure adherence to all relevant laws, regulations, and professional guidelines, including controlled substances laws.
Record keeping: Maintain accurate patient records, including prescriptions, medication therapy management services, and any other pertinent information.
Continuous education: Stay up-to-date with advancements in pharmaceutical science, treatment protocols, and industry best practices.
2. Minor Responsibilities
Maintain daily workflow of the department, giving direction to support staff
Fill in as needed at alternate pharmacy locations

Requirements
Education & Licensure:
Graduate of an ACPE accredited School of Pharmacy with a Pharmacy degree
Current Wisconsin Pharmacist License
BLS certification required
ACLS prep courses (medication and EKG) required within 6 months of hire
Experience:
Prior hospital pharmacy experience required, 3 – 5 years preferred
Pharmacy Residency completion in lieu of experience

Prolonged periods of sitting at a desk and working on a computer
Prolonged periods of standing, bending, and reaching
Must be able to lift up to 50 pounds at times
