Position Summary:
The Clinical Program Coordinator has a primary role in the oversight, coordination, and implementation of multiple clinic-based programs at Roots Community Health Center. Assigned programs may vary based on organizational priorities and funding; examples include HIV Prevention and Management; Substance Use Disorder Treatment for opioid and stimulant use, Hepatitis C detection & treatment; Diabetes Self-Management Education & Support, Sexually Transmitted Infection (STI) detection and treatment, COVID, and Complex Care. This position is based at the Main Adult Clinic with travel to other Roots sites, as needed. This position is focused on our program for Substance Use Disorder Treatment for opioid and stimulant use.
Duties and Responsibilities:
Program Coordination & Communication
Coordinate activities of navigation, outreach, and patient services staff to ensure timely and high-quality service delivery.
Schedule, attend, and contribute to internal and external meetings related to program development, reporting, and community advocacy.
Ensure timely dissemination of updates on program offerings, eligibility requirements, and referral pathways to internal teams, particularly clinic and navigation staff.
Identify and troubleshoot gaps in communication or service delivery across the continuum of care.
Receiving and managing calendar invites and communications related to protocol development, referral tracking, program scheduling, and data collection efforts.
Protocol Development & Program Design
Schedule in-field shadowing with Dr. Williams and members of the STOMP team to observe current service delivery models.
Observe Program Specialist outreach and navigation practices (ad hoc) and use findings to document and refine step-by-step protocols for service delivery.
Draft, refine, and implement clear protocols, staff training materials, and communication tools that support effective program implementation and continuity.
Contribute to overall clinical strategy development and program innovation.
Client Data Tracking & Evaluation
In partnership with Program Administrator and relevant data staff, develop and refine client data tracking system.
Track and ensure documentation of all program/contract deliverables.
Identify and report data collection issues or limitations to the Program Administrator, Program Manager and Director in a timely manner.
Monitor and ensure consistent, accurate, and complete documentation of client services and program activities.
Client Engagement & Outreach Support
Support Program Specialist with transportation for client engagement and retention as needed, coordinating schedules accordingly.
Serve as the Roots staff member authorized to operate a Roots vehicle for client transport.
Lead or support patient recruitment, enrollment, and outreach efforts to ensure program participation and engagement, particularly among high-priority populations.
Collect and analyze feedback from clients and community members to inform improvements in service delivery and program structure.
Quality Improvement & Staff Support
Identify training gaps and areas for staff development; implement or elevate needs to leadership as appropriate.
Assist in the preparation of data and narratives for program reports to funders and internal leadership.
Participate actively in staff and agency meetings and ongoing professional development.
Requirements:
Competencies:
Interest and willingness to learn about basic disease management concepts and health messaging specific to the programs.
2 years of experience working in a non-profit organization, or a community clinic preferred.
Excellent communication skills (written and oral) with comfort in presenting training and motivating change among various stakeholders.
Ability to discuss stigmatized health conditions (Hep C, HIV, opioid dependence) and behaviors (needle sharing, unprotected sex) in a nonjudgmental manner.
Strong organizational skills.
Proficient in computer use, including MS Office and G-Suite, and ability to learn new Electronic Medical Records (EMR) and proprietary database systems.
Interest in working with low-income individuals, especially people of African descent, in an effort to empower the community and eliminate disparities.
