Job Summary:

The Supervisor HCBS

has supervisory responsibility for assigned staff of the Home & Community Based Services Program, including implementation of HCBS program requirements as promulgated by the Department of Health & Human Services (DPHHS) contract and Agency policy. The position has operational responsibility for the quality of services provided to clients, the safety of assigned employees, and compliance with applicable laws and regulations.

Essential Duties and Responsibilities:

Oversees day-to-day operation of

assigned HCBS

staff in compliance with authorized Medicaid budget.

Assists with

maintaining a full client load at Medicaid-authorized levels through community outreach and marketing efforts. Prioritizes continuity and continuation through relationship-building with State Medicaid Staff.

Oversees care plans, including cost estimate reviews, compliance requirements, discharge planning and documentation.

May handle an appropriate case load of HCBS clients as determined by the HCBS Manager.

Hires, orients, and evaluates staff competency and performance, and provides training, coaching, counseling and discipline as warranted. Must have the ability to work with a remote team who work from home; maintain high morale; prioritize communications and team building.

Accompanies staff periodically on client visits to ensure high quality service.

Monitors reports and statistics for productivity and efficiency. Develops and implements action plans to address any issues identified.

Develops, monitors and enforces compliance with applicable policies, procedures, regulations and laws.

Establishes safe work practices and work environments to protect the safety and well being of employees.

Performs other duties as assigned by supervisor.

Minimum Qualifications:

Qualifications include Bachelors Degree in Social Work or in a health-related field;

2 years of experience in a related field, supervisory experience strongly preferred; a valid drivers license, appropriate auto insurance and reliable transportation.
