Day Shift:

Friday - Monday

8:00 AM - 5:00 PM

Coordinate and Manage Patient Scheduling: Schedule and adjust patient visits in a timely manner to ensure continuity of care, compliance with regulations, and patient satisfaction. Serve as a liaison with patients, clinicians, contract agencies, and other departments to manage visit frequency, coverage assignments, and scheduling changes.

Support Clinical Staff and Operations: Provide scheduling support to field clinicians by managing visit assignments, responding to inquiries, adjusting schedules as needed, and helping to balance workloads and overtime. Attend care team meetings and maintain communication to support care planning and delivery.

Facilitate Communication and Problem Solving: Act as a key communication hub between patients, clinicians, supervisors, and support teams. Troubleshoot scheduling issues, assist with referrals, address patient concerns, and ensure accurate documentation and efficient use of systems like Epic.

Education:

Associate degree in business, health administration, related field, or equivalent experience.

Experience:

Minimum 2 years of scheduling or customer service experience.

License(s)/Certification(s):

Valid driver's license when diriving any vehicle for work-related reasons.
