Annual Salary $91,840.35 to $124,254.59

The FTE for this role is an annualized .775 FTE and follows the school schedule.

Job Description:

The Advanced Practice Provider (School-Based Health Center) provides medical care to elementary and secondary school students in accordance with the State of Washington Department of Licensing and professional scope of practice. The APP is responsible for providing medical care to school-aged children within SBHC locations in accordance with the State of Washington Department of Licensing and their professional scope of practice. The APP is responsible for the documentation of care provided as well as indirect care related to communicating with parents/guardians and/or co-located service providers as appropriate and required. SBHC providers typically operate without a Medical Assistant and are responsible for vaccine compliance, lab result notification, and supporting clinic and equipment formulary and inventory. This position is solely dedicated to the CHC school-based health center (SBHC) locations with a clinical schedule mirroring the supported school(s) operating days. The position may require travel between sites, flexible hours, and the potential for evening and weekend events.

High school graduate or equivalent.

Bachelor’s degree as a Physician Assistant or

Registered Nurse with a Mater degree or post Master’s certificate as an Advanced Registered Nurse Practitioner (ARNP). Preferred:

Education and training in specialized care such as family practice, internal medicine, pediatrics, obstetrics or prenatal Experience

Experience as a practicing clinician (1 year).

Electronic Health Record (EHR) and/or Electronic Practice Management System experience (1 year).

Experience working with low-income, diverse populations (1 year). Preferred:

Experience in a multiple provider dental practice.

Digital imaging familiarity.

CDT-5 coding experience.

Healthcare information systems, such as electronic health record and practice management systems experience.

Working with low income, multi-ethnic populations. Credentials

Physician Assistant license, with prescriptive privileges with the State of Washington, or

Advanced Registered Nurse (ARNP) license with prescriptive privileges with the State of Washington

If a Physician Assistant, having PA certification by the Nation Commission on certification of PA (NCCPA)

If an Advanced Registered Nurse Practitioner, ARNP certification by the American Nurse Credentialling Center (ANCC) or American Academy of Nurse Practitioners (AANP).

DEA controlled substances registration.

Basic Life Support for Healthcare Providers (CPR/AED) certification - must be obtained within 45 days of employment.

Successful completion of CHC’s credentialling and privileging review upon hire and recredentialling as needed. Other:

Driver’s license with the State of Washington

Motor vehicle insurance liability policy, a certificate of deposit, or a liability bond to the required limits

Job Specific Functions/Performance:

Provides preventive, diagnostic, and therapeutic medical care to school aged children and their families in accordance with the State of Washington Department of Licensing and within their scope of practice and in alignment with CHC requirements.

Excels in working with school-age children around their healthcare needs.

Communicates with parents/guardians regarding student/patient care, as appropriate.

Ability to develop and maintain relationships with community partners.

Coordinates delivery of care with appropriate clinical staff and community partner staff to ensure quality of care is met.

Instructs patient and/or family regarding medications and treatment instructions. Promotes medical care through direct patient care and education.

Identifies special needs of community partner patients and works with staff to address those needs.

Adheres to attendance standards in order to perform the job functions for daily operations and/or continuity of patient care.

Provides family planning / STD assessment, diagnosis and treatment to male & female adolescents.

Provides education on a variety of health topics to student groups in collaboration with school staff.

Works with the clinic team and school faculty to promote the services of the SBHC in various venues on campus.

Participates in Quality Improvement and Utilization Management programs and activities.

Documents encounters in an accurate and timely manner.

Works collaboratively with the outreach department to build relationships with people experiencing obstacles in obtaining healthcare, provides advocacy, and assists with connection to health services and other social services as needed.

Performs all duties and responsibilities contained in the job description for a Nurse Practitioner or Physician Assistant, depending on specific credentials.

Adheres to attendance standards in order to perform the job functions for daily operations and/or continuity of patient care.
