Job Summary:

Provide home health occupational therapy services to patients, helping them improve their ability to perform daily activities.

Responsibilities:

Conduct assessments and develop individualized treatment plans.

Administer occupational therapy treatments in patients' homes.

Monitor and document patient progress.

Educate patients and families on therapeutic exercises and activities.

Collaborate with healthcare team members.

Qualifications:

At least 6 months of active or recent experience, preferably in home health.

Valid Occupational Therapist license in the State of California.

Strong communication and interpersonal skills.

Ability to work independently.
