Your Role:

As a Weekend Physical Therapist, you will provide vital therapy services to patients transitioning from inpatient facilities. You will assess, plan, implement, and evaluate total patient care while coordinating with care teams to deliver quality treatment based on physician orders. Your focus on patient needs and compliance with industry standards will help ensure the best outcomes.

Key Responsibilities:

Perform thorough patient evaluations to assess functional abilities and safety in their home environment.

Provide ongoing care and monitor progress through regular reassessments.

Document treatment plans and patient responses accurately in clinical records.

Collaborate with health team personnel to ensure comprehensive patient support.

Customize therapy interventions using a range of techniques to optimize patient recovery.

Adhere to documentation timelines to maintain compliance with agency standards.

Participate in team meetings and contribute to clinical record reviews.

Qualifications:

Bachelor’s degree in Physical Therapy with at least one year of relevant experience

Licensed as a Physical Therapist in the state

Current CPR certification

Valid Driver's License
