POSITION SUMMARY:

Joseph P. Addabbo Health Center is looking for compassionate, energetic, and committed professional(s) to help patient (s) access HIV care services and remain engaged in their own treatment. Patient Navigator (s) have a close understanding of the community served (PLWH, Communities of Color, LGBTQ communities, etc.) and use best practices to assess the needs of PLWH and connect them with internal and/or community services and resources. Ideal candidates apply culturally responsive approaches to healthcare and health equity by engaging diverse groups (culture, age, gender identity, sexual orientation, disabilities, races, and religious beliefs). The Patient Navigator will work closely with infectious disease providers and other clinical staff as well as case managers, and support staff across the HIV care continuum to assist PLWH receive comprehensive quality of care.

RESPONSIBILITIES:

· Educate and link patients to programs or outside services to support maintenance within their home and community.

· Builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities, such as outreach, community education, informal counseling, social support, and advocacy.

· Utilize strategies such as motivational interviewing to encourage individuals to start and remain in care

· Contact patients that have cancelled or no-show appointment records to reengage into care.

· Provide HIV education including prevention, chronic disease management, HIV treatment and medication adherence.

· Follow-up with new/returning patients after their initial appointment/ returning appointment regarding follow-up questions, concerns, or barriers to care.

· Follow-up with patient who have newly been prescribed anti-retroviral therapy medications or who have had changes in medications regimen.

· Monitor on-going visits to track patient progress and efficacy of their developed care plan.

· Collaborate with multi-disciplinary team to ensure the best quality care is provided to each patient

· Maintain and update patient records as needed including timely documentation in the electronic health system (E-shares and ECW) and or other databases in accordance with HRSA, NYC DOHMH, and JPA’s clinical guidelines, policies, and procedures.

· Stay current with latest advancements in HIV/AIDS treatment research and regimens through ongoing training and technical assistance.

· Possess driver’s license and/ or able to navigate travel on MTA/public transportation system to conduct field visits.

· Serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural responsiveness of services delivered.

· Support continuous quality improvement activities through case conferencing, clinical supervision, and or other clinical quality management efforts.

· Flexibility to work on some weekend.

MINIMUM QUALIFICATIONS:

· Bachelor’s Degree in public health, nursing, or social work, and or five plus years in related work experience

· Minimum 2 years’ experience working with People Living with HIV (PLWH)

· Experience working with electronic health records and ability to maintain hard copy case records

· Experience working with priority populations including but not limited to (PLWH, Communities of Color, LGBTQ+, etc.)

· Must demonstrate broad understanding of HIV epidemic (history, priority populations, transmission, prevention, treatment, sociopolitical issues, and quality improvement).
