Our Transition Youth and Young Adult Services team at Southeast Healthcare Services is seeking a dedicated and qualified professional to provide community-based services to youth ages 14–18. This role is focused on supporting participants as they develop and strengthen independent living skills and employment readiness.
Key Responsibilities:
Conduct initial and ongoing assessments of participants’ independent living skills, vocational strengths, and interests
Develop individualized service plans aimed at achieving participants' goals in areas such as education, employment, and daily living
Provide hands-on support and coaching to assist youth in obtaining and maintaining employment
Facilitate access to resources and create learning opportunities that promote self-sufficiency
Collaborate with service providers, families, and systems of care to ensure coordinated support

Salary range based on education and related work experience = $35,000 - $42,660.
