POSITION SUMMARY
The MDS Coordinator is accountable for editing, locking and the transmission process of all MDS assessments. The overall responsibility of this position is to help assure timely completion of the MDS with a focus on accuracy, case mix maximization, and regulatory compliance. He/she oversees the clinical and regulatory component of MDS completion related to reimbursement. This position also coordinates care plan completion with the Interdisciplinary Team (IDT), and renders clinical support to nursing staff to enhance accurate assessments, care plans and excellent quality of care.
RESPONSIBILITIES
Timely and Accurate MDS Completion: Performs admission chart reviews, selects ARDs, inputs ICD-10 codes, completes MDS and CAA documentation using comprehensive resident data, and ensures timely submission per regulatory standards.
Medicare and PDPM Oversight: Manages PDPM processes, monitors Medicare eligibility, completes necessary billing paperwork, and leads weekly IDT case management meetings to support Medicare and Managed Care case mix.
Interdisciplinary Collaboration: Works closely with the IDT to ensure care plans are accurate, comprehensive, and completed on time; attends and contributes to QAPI meetings and quality improvement efforts.
Quality Assurance and Reporting: Monitors quality measures, completes facility QAPI reports, participates in the monthly Triple Check process, and identifies MDS-related issues through validation reports.
Professionalism and Communication: Maintains a positive, professional image; upholds resident confidentiality; communicates courteously with staff, residents, and families; follows workplace conduct policies and procedures.
Compliance and Continued Education: Keeps licensure current, attends required meetings and in-services, follows state regulations and Monarch policies, and supports a culture of inclusivity, safety, and continuous improvement.
QUALIFICATIONS
Must be a Licensed Registered Nurse in the state of Minnesota
Must have successfully completed or seek certification in Cardiopulmonary Resuscitation and emergency care for the health care provider.
Must possess the ability to communicate verbally and in writing when directing the Resident Assessment Instrument (RAI) process and when performing and assessing care for residents.
Experience in healthcare, skilled nursing facility or assisted living community
Experience with MDS and Resident Assessment Instrument (RAI)
