Position Summary:

The Nurse Practitioner (NP) Supervisor for Home Care is a clinical leadership role responsible for overseeing a team of advanced practice providers and ensuring high-quality, patient-centered care across in-home health services. The NP Supervisor provides direct patient care while also supporting staff performance, clinical standards, regulatory compliance, and coordination of care. This role combines clinical expertise with operational oversight to ensure excellence in home-based care delivery.

Key Responsibilities:

Clinical Care & Oversight:

Provide direct medical care to patients in their homes, including assessments, diagnoses, treatment plans, medication management, and follow-up.

Serve as a clinical resource and mentor to Nurse Practitioners, ensuring adherence to best practices and clinical protocols.

Perform joint visits and chart reviews to maintain quality and compliance with care standards.

Leadership & Supervision:

Supervise a team of Nurse Practitioners and possibly Physician Assistants, including onboarding, scheduling, and performance evaluations.

Support continuing education, skills development, and credentialing of clinical staff.

Facilitate case conferences and interdisciplinary collaboration with nursing, therapy, social work, and home health aides.

Regulatory & Quality Assurance:

Ensure documentation and care delivery meet state, federal, and agency guidelines (e.g., CMS, Joint Commission).

Lead quality improvement initiatives and participate in audits, surveys, and clinical reviews.

Develop and implement clinical policies and procedures specific to home care.

Coordination & Communication:

Act as a liaison between the home care agency and external partners, including hospitals, primary care providers, and specialists.

Collaborate with case managers and home health nurses to facilitate timely care transitions and reduce avoidable hospitalizations.

Address escalated clinical concerns from families, caregivers, or other staff.

Qualifications:

Education: Master’s or Doctoral degree in Nursing from an accredited Nurse Practitioner program.

Licensure: Active Nurse Practitioner license in the state of New York; board certification in Family, Adult-Gerontology, or relevant specialty.

Experience: Minimum of 3 years of NP experience, with at least 1–2 years in home care, palliative care, or geriatric practice preferred.

Prior supervisory or leadership experience strongly preferred.

Knowledge of EMRs and mobile health documentation platforms.Company Description
