The wellness specialist is responsible for providing access to community resources, daily living skills training and support for those living in our supportive housing programs.
The wellness specialist is a critical resource and advocate for our individual's.
Some of the responsibilities of the Wellness Specialist include: Facilitate individual's linkage and access to services available the community.
These include case management and clinical services, day programs, vocational opportunities, community integration, entitlements Outreach to community services, agencies and other wellness providers to bring wellness to the individuals as needed.
Development of a comprehensive needs assessment.
Schedule meeting with individuals to develop skills and review progress.
Provide medication management and support for all individuals in program.
Engage in recovery planning through the use of Wellness and recovery initiatives.
Provide daily living skills training (e.g.
medication monitoring, personal money management, community integration etc...)
