Experience: Minimum 1 year of Certified Home Care experience and 1 year of Case Management experience

License: Current licensure in the state of Massachusetts

Must-Have:

Reliable, independent means of transportation to make home visits.

Must be willing to work weekends, holidays, and on-call in accordance with department needs and rotation.

Weekend rotations are required every 6 weeks.

Description:

Provide clinical services to patients from birth to over 100 years in their homes.

Assess patient conditions and implement care plans.

Provide ongoing education and support to patients and their families.

Conduct admissions processes and case management responsibilities in accordance with policies and standards of practice.

Utilize thorough and timely electronic documentation and interdisciplinary communications.
