Grow With Us!Presbyterian Village North is immediately hiring a LVN Supervisor/Nursing Care Coordinator for Weekend Doubles in the Assisted Living Department. Plus $500.00 Sign On Bonus!Saturday and Sunday 7am - 11pm (6:45am-11:15pm).

ESSENTIAL DUTIES:

\tModels a service and solutions oriented attitude towards others, takes ownership in solving problems and takes the initiative to make things better, is friendly and courteous to residents, patients and co-workers

\tEnsure resident/patient, visitor, and co-worker safety by remaining aware of and reporting any new or existing safety hazards

\tMaintain good attendance record

\tRecruit, hire, train, coach, and evaluate departmental staff (management staff only)

\tAssess residents and completes MDS according to facility policy

\tDevelop and revise resident service plans to reflect residents current status

\tMonitor residents for changes in condition, ensure MD, and family, a are notified and appropriate documentation is reflected in the medical record

\tAssess residents initially and annually and complete resident service plans on an annual or as needed, basis

\tChair the Interdisciplinary Service Plan Meeting

\tMaintains the Residents Roster and Acuity Levels within the department (i.e., State Boards, QA monitor)

\tReview and follow-up all consultant reports in a timely manner

\tReviews monthly physicians orders and ensures accuracy

\tMonitors psychotropic behavior sheets and makes recommendations as needed

\tComplete performance evaluations of all licensed nurses, mediation aides and CNA assigned to AL

\tNotify Director of Assisted Living Services of incident reports concerning abuse

\tThoroughly investigate any complaints and concerns and report to Director of Assisted Living Services

\tAttend all assigned committee meetings

\tReview incident report documentation and ensures follow-up according to facility policy

\tMake rounds to verify clinical compliance and quality of care and provide coaching and disciplinary action as indicated within the facilities policies and procedures

\tEnsures all Life Enrichment programs/QA monitors are implemented on assigned units

\tPerform change nurse duties, in cases of staffing emergencies, as required to meet the residents immediate needs

\tOther duties as assigned, including filling in as Nurse duties required, as needed

MINIMUM QUALIFICATIONS:

\tValid Texas LVN license

\tDemonstrated supervisory and managerial skills

\tKnowledge of current professional nursing theory and practice

\tExcellent technical, assessment and documented skills

\tAbility to organize workflow, respond to emergencies and note detail

\tAbility to maintain a positive, stable disposition in performing work in association with staff and residents

\tAbility to maintain a high level of performance in an environment of frequent demands and changes

\tAbility to read, write and effectively communicate in English

\tExcellent verbal and written communication skills

\tAbility to perform Essential Duties

\tGenuine caring for and interest in the care of mature adults, persons with handicap and hospice patients

\tAbility to perform work tasks within the Physical Demand Requirements

\tAbility to comply with the Resident Bill of Rights and Employee Responsibilities

\tAbility to comply with organizational and departmental safety policies and procedures
