Intake Coordinator Needed
CareFor is seeking a compassionate and detail-oriented Intake Coordinator to serve our Care Management team.
The Intake Coordinator will primarily focus on initial engagement with clients, assessing patient needs, facilitating the referral process, and ensuring the appropriate care and support within the CareFor service continuum. They will provide overall planning, direction, and coordination of patient care including intake, scheduling and completion of administrative duties.
The Intake Coordinator will:
Facilitate the referral intake process, coordinate care with the interdisciplinary team, the patient, their family, and the referral source.
Determine the scope and frequency of initial services needed based on the patient’s condition.
Maintain effective communication with patients, families, team members, and other healthcare professionals.
Gather patient demographics, clinical support documentation and insurance information as applicable.
Assure the development, implementation, and updates of the individualized plan of care.
Receive and appropriately enter/document new referrals in a timely manner.
Accurately document patient information and complete entry and workflow tasks and other assignments in the Electronic Medical Record (EMR).
Work with the patient and referral sources to coordinate care and schedule initial visits.
Ensure the scheduling of initial visits and other tasks for the assigned service-level team.
Participate in information management and quality assurance performance improvement (QAPI) activities.
Ensure billing information is entered in patient files for billing rate accuracy and coordinate with accounting as needed.
Maintain a high level of integrity and discretion in handling sensitive and confidential information.

Ability to communicate clearly and effectively with clients, families, and healthcare professionals.
Ability to assess and address clinical issues in a timely manner, ensuring proper care for the client.
Demonstrated empathy and the ability to offer emotional support to clients and families.
Strong documentation skills and meticulous attention to detail to ensure accurate patient records.
Ability to adjust to a variety of client needs, care environments, and medical situations.
Ability to understand of and adhere to HIPAA regulations and always maintain the confidentiality of patient information.
Ability to work independently and as part of an interdisciplinary team.
One to three years of experience as a Social Worker in a clinical role (preferred).
Experience in home health or hospice care (preferred).
