Qualifications: * Minimum of high school diploma; Associate's degree or higher preferred * Experience in a social services or case management role, preferably in a healthcare or community-based setting preferred.
* Experience in working with social determinants of health (SDOH) and understanding their impact on overall health.
* Familiarity with community resources, advocacy, and public assistance programs (e.g., housing, food security, employment). * Experience in case management, resource coordination, and patient-centered care is preferred.
Skills and Abilities: * Strong interpersonal and communication skills, with the ability to build rapport with patients, families, and community partners.
* Excellent organizational skills, with the ability to manage multiple priorities and deadlines.
* Critical thinking and problem-solving abilities to identify and address complex social needs.
* Proficiency in using Electronic Health Records (EHRs) and other healthcare management systems.
