Medical Director of Nephrology

AbsoluteCare is on the lookout for an enthusiastic physician leader who is passionate about improving the lives of our medically complex and socially vulnerable members affected by kidney disease. Due to factors such as trauma and poverty, many individuals in our community are living with kidney disease, and we believe in making a significant impact.

The Medical Director of Nephrology (MDN) will play a key role in Population Health Management for thousands of members coping with Chronic Kidney Disease (CKD). In this pivotal position, you will collaborate closely with our CKD project manager and integrate efforts across our clinical enterprise to leverage data analytics and performance metrics. This will allow you to monitor patient outcomes, identify improvement opportunities, and spearhead quality initiatives, making your mark in our rapidly growing organization.

Key Responsibilities:

The Medical Director of Nephrology will serve as a clinical leader, providing expert guidance to ensure high-quality, evidence-based nephrology care, promoting affordability while decreasing the total cost of care under value-based principles.

Lead a transitional care team while cultivating strong relationships and communication pathways with nephrologists, primary care physicians, hospital systems, payers, and essential stakeholders to ensure seamless, high-quality care delivery.

Oversee clinical, utilization management, and financial outcomes for total members attributed to AbsoluteCare, regardless of where they receive primary care services.

Facilitate and lead discussions with both AbsoluteCare and Non-AbsoluteCare providers on optimal care delivery, utilization, and care coordination across various healthcare settings including hospitals, emergency departments, skilled nursing facilities, and more.
