About the Role
As a Nurse Practitioner with Matrix, you will conduct comprehensive health assessments during visits that last up to one hour. These assessments offer invaluable insight into members' overall health, which can be challenging to capture in standard office visits. Your responsibilities will include:
Performing adult and geriatric assessments covering medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and documentation per Matrix protocols.
Ordering and administering point of care and lab-based testing as needed.
Collaborating with physicians, social workers, family members, and other healthcare professionals as appropriate.
Working together with Primary Care Physicians (PCPs) on patient education and follow-up as needed.
Providing services at the member’s residence, whether in homes or Skilled Nursing Facilities.

Work Schedule: Primarily Monday to Friday from 8:00 AM to 5:30 PM, with flexibility to conduct assessments on weekends as necessary.
Competitive Compensation: Base salary ranging from $105,500 - $168,400 based on experience and market analysis.

Skills & Experience Required:
Master's Degree in Nursing or relevant field with satisfactory completion of NP licensure.
Current NP licensure, including prescriptive authority or capacity to obtain it.
Board certification from AANP, AACN, or ANCC in relevant specialty areas.
Current BLS, ACLS, or CPR certification.
A minimum of one year experience as a Nurse Practitioner.
Ability to travel up to 80-90% of the time, including overnight stays and weekends across the assigned state and potentially neighboring states.
Valid driver's license and proof of automobile insurance.
Strong computer skills and familiarity with medical record software.
Excellent verbal and written communication skills.
