About the Role:

As a Licensed Vocational Nurse (LVN) at Silver Spring Health & Rehabilitation, you will play a critical role in delivering compassionate and high-quality nursing care to our patients. Your primary focus will be to support the healthcare team by providing direct patient care, monitoring patient health, and assisting with medical procedures under the supervision of registered nurses and physicians. You will ensure that patients' physical and emotional needs are met, contributing to their overall well-being and recovery. This role requires meticulous attention to detail, strong communication skills, and the ability to work effectively in a fast-paced healthcare environment. Ultimately, your efforts will help maintain a safe, supportive, and healing environment for all residents and patients within our facility.

Minimum Qualifications:

Current and valid Licensed Vocational Nurse (LVN) license in the state of Texas.

High school diploma or equivalent required; completion of an accredited vocational nursing program.

Basic Life Support (BLS) certification from the American Heart Association or equivalent.

Demonstrated ability to perform nursing duties in a long-term care or rehabilitation setting.

Strong communication skills and the ability to work collaboratively within a multidisciplinary team.

Preferred Qualifications:

Experience working in a skilled nursing facility or rehabilitation center.

Additional certifications such as IV therapy, wound care, or geriatric nursing.

Familiarity with electronic health record (EHR) systems and computerized documentation.

Knowledge of state and federal healthcare regulations related to long-term care.

Proven ability to provide culturally competent care to a diverse patient population.

Responsibilities:

Provide direct nursing care to patients, including administering medications, monitoring vital signs, and assisting with daily living activities.

Collaborate with registered nurses, physicians, and other healthcare professionals to develop and implement individualized care plans.

Document patient information accurately and timely in medical records, ensuring compliance with healthcare regulations and facility policies.

Observe and report changes in patients’ conditions to the healthcare team promptly to facilitate appropriate interventions.

Assist with wound care, catheter care, and other clinical procedures as delegated by supervising medical staff.

Educate patients and their families on health maintenance, medication management, and post-discharge care instructions.

Maintain a clean, safe, and organized work environment in accordance with infection control and safety standards.

Skills:

The required skills for this role include clinical nursing competencies such as medication administration, patient assessment, and basic nursing procedures, which are essential for delivering safe and effective care daily. Strong communication skills enable you to interact clearly and empathetically with patients, families, and healthcare team members, ensuring coordinated care and patient understanding. Preferred skills like proficiency with electronic health records streamline documentation and improve information sharing within the care team. Additionally, specialized knowledge in wound care or IV therapy enhances your ability to address complex patient needs, contributing to improved patient outcomes and satisfaction.
