We're seeking a compassionate and resourceful Service Coordinator to join our team. In this role, you will connect residents with programs, services, and community resources that enhance their quality of life and support their ability to age in place. Working closely with residents, local service providers, and our property management team, you'll play an essential role in helping residents live independently, safely, and with dignity.
Key Responsibilities
Resident Programs & Community Engagement
Coordinate group programs and activities that promote resident engagement, education, and well-being.
Serve as a liaison between residents and community agencies; develop partnerships with local service providers and negotiate low-cost or discounted services.
Monitor the quality and effectiveness of community services and maintain detailed documentation of resident progress.
Develop and maintain a Resource Directory of community and state service providers (healthcare, family support, disability resources, etc.).
Organize educational events on health, life skills, and community resources.
Implement on-site or mobile health services and screenings.
Encourage resident participation in both on-site and community-based programs.
Promote volunteer involvement following company policies and procedures.Individual Resident Support
Provide quality customer service to all residents, addressing concerns and assisting with everyday needs.
Help residents access community resources, including transportation, financial assistance, personal services, meal programs, and counseling - in compliance with HUD guidelines.
Treat all residents with respect and professionalism while reporting any complaints, concerns, or allegations of abuse to appropriate supervisors and authorities.Administrative & Compliance Duties
Maintain accurate documentation of all resident interactions and services provided.
Complete required forms, Care Plans, and Quarterly Monitoring reports for residents utilizing service coordination.
Submit Semi-Annual Performance Reports to HUD to maintain program funding.
Manage time efficiently, work both independently and collaboratively, and respond promptly to all communications.
Follow all company policies, federal and state regulations, and uphold corporate compliance standards.
Participate in all required professional training.Qualifications
Experience in social services, case management, or a related field preferred.
Strong interpersonal and communication skills with a compassionate approach to resident care.
Ability to work independently, prioritize tasks, and maintain confidentiality.
Knowledge of HUD programs and community resources is a plus.
Proficient in basic computer applications and documentation systems.
