Overview
The Physical Therapist provides rehabilitative care (physical therapy) for home health patients needing assistance and training in ambulation, transferring, gait training and home exercise programs. The Physical Therapist works as part of the interdisciplinary home care team including the Registered Nurse, Physical Therapist, Speech Therapist, Social Worker and Home Health Aide.

Shift: PRN
Responsibilities
Provide physical therapy services to patients according to a plan of care. This may include assessing and evaluating therapeutic/rehabilitative/functional status and participation in developing the total plan of care as well as providing direct physical therapy treatment.
Assess patients for muscle strength, mobility, gait, ROM and transfer capabilities.
Instruct patients, families and caregivers in the use and care of therapeutic appliances.
Report patient's reaction to treatment or changes in care to patient's physician as needed and appropriate.
Initiate physical therapy programs and instruct other personnel, patient and/or family/caregivers in their therapy program, as well as the goals of therapy.
Supervise the Physical Therapist Assistant (PTA) as necessary.
Participate in Otterbein Universal Home Health Quality Assessment/Performance Improvement program.
Confer with the attending physician, home care team members and patient and/or family/caregivers to promote coordinated and comprehensive care.
Prepare and submit clinical and progress notes in a timely manner and in accordance with Otterbein Universal Home Health, state and federal guidelines and regulations.
Education and Experience
Education: Bachelor's degree or master's in physical therapy.
Experience: Minimum 2 years of physical therapist experience with community / home health preferred.
Licensure, Registration, and/or Certification: Current license as a Physical Therapist from the State of Ohio or made application for same. Must have a valid Ohio driver's license and access to reliable transportation. Current CPR certification required.
Complete a minimum of twelve (12) hours of in-services per year.
