Position Summary:

This position is responsible for assisting those overcoming addictive behaviors. A person in this position accomplishes this by utilizing personal experiences battling addictive behaviors and providing a wide variety of services to clients to ensure compliance with scheduled follow up appointments with medical providers. This position operates under minimal supervision and must exercise tact, judgment, and confidentiality in contact with the public, clients, and families, as well as fellow associates of the Center. Work is very broad in scope and requires a high level of professionalism.

Position Responsibilities include but are not limited to:

Demonstrate the ability to complete client assessments, including OBHP intakes (based on education and training), evaluating a client's ability to perform daily living activities and independent living skills, identify presenting problem(s), history, medical information, family involvement, psychosocial/psychosexual information, and other pertinent information to form an appropriate clinical impression including substance use/abuse and abuse/neglect.

Demonstrate a basic understanding of treatment plans and service lines in which client has been referred to within the Center.

Document timely, accurately and in accordance with Center policies utilizing current systems and practices.

Document accurate information, which reflects client progress based on the appropriate therapeutic modality. Therapeutic services will be time limited and specific to the presenting problem(s) of each assigned client.

Provide input as to the client's progress towards treatment plan goals and objectives, communicating pertinent information regularly with the primary case manager so such information can be presented at staffing.

Provide direct client care and maintain an RVU expectation as assigned by agency

Demonstrate competency and understanding of billing codes as it pertains to the use of peer billing codes and MRO billing codes.

Provide direction and assistance to clients to assist them on overcoming addictive behaviors.

Facilitate follow up recommendations from the medical and mental health providers.

Assist the client by connecting them to services within and outside of the Center as needed.

Assist clients to become self-reliant by empowering them to coordinate their own medical issues, teach self-advocacy skills, provide outreach, aid in goal setting, and assist in identifying a natural support system.

Use evidence-based interventions with all clients to aid in developing increased skills for problem solving relapse prevention planning and anti-stigma education and support.

Provide phone calls and face-to-face contact to ensure clients do not have any question regarding their care or treatment.

Assist in identifying and reducing barriers to follow up care.

Provide on-going assistance to clients to reintegrating them back into the community; meeting with them regularly as defined by the department/supervisor.

Support clients in engagement of recreational and social experiences to enhance social and community connectedness.

Maintain mandatory Center in-service training requirements.

Provide client transportation as needed.

Abide by all Center policies and procedures.

Maintain positive working relations and rapport with the communities served.

Starting and growing base wage opportunities up to $28.25/hour!

Additional Annual Compensation and Incentive Opportunities!

Four-day (32 hour) work week

Make up to an additional $625 per week by participating in Mobile Crisis On-Call opportunities in eligible counties

Eligible for $150/month ($1,800 annually) in student loan repayment, available at hire

Earn $750 for every person you refer who is hired and remains employed for 6 months and receive an additional $500 every year as long as you both remain employed

Bilingual wage premiums available for individuals fluent in Spanish or Burmese

Qualifications

Requirements:

Background Checks:

4C Health is a Drug-Free Workplace employer. Candidates must be willing to submit to a pre-employment drug screen.

Candidates must be willing to submit to comprehensive background checks, including but not limited to criminal and child protective services checks.

Education:

High School graduate or GED

A degree or further education in social services, psychology or mental health preferred but not required.

Experience:

Lived experience with mental health and/or addiction services required and been in recovery (2) or more years.

At least one (1) year of full-time experience in mental health, childcare, or another related field preferred.

Skills/Knowledge:

Works well under supervision as well as independently. Organizes/manages time and daily activities with little direct supervision.

Ability to think critically and analyze information/situations for most appropriate response.

Strong organizational skills and detail oriented.

Manages complex tasks and requests supervision when necessary.

Able to utilize databases and/or other electronic means to perform duties.

Models professionalism, teamwork, and communication internally and externally.

Ability to maintain confidentiality.

Maintains positive working relations and rapport with the community.

Ability to model daily living skills.

Must be at least twenty-one (21) years of age.

Must have a valid Indiana Driver's License and maintain vehicle insurance requirements per Center policy.

Must have current CPR certification or ability to obtain within 30 days.

Must have current CPI certification or ability to obtain within 30 days.

Must have current CAPRC or CHW/CRS or ability to obtain within (1) year Knowledge of community resources
